- Known or suspected prostate cancer
- Male breast cancer
- Untreated severe obstructive sleep apnea
- Elevated hematocrit or polycythemia above the clinician's threshold
- Actively trying to conceive — testosterone suppresses sperm production
- Pregnancy, breastfeeding, or possible pregnancy in a household member exposed to topical forms
- Elevated PSA or a family history of prostate cancer
- Heart failure, recent cardiovascular event, or uncontrolled hypertension
- History of blood clots, deep vein thrombosis, or pulmonary embolism
